General instructions to clinic patients
Base Hospital Gampola conducting range of clinics to provide better service to
patients. These clinics provide follow up care to patient who were treated in
house, continuation of treatment for chronic diseases like diabetes, ischemic
heart diseases, hypertension, chronic obstructive pulmonary disease (COPD),
Bronchial asthma, rheumatoid joint diseases, etc.
Patients who wish to attend to a clinic for the first time at BH Gampola need to
have a referral letter. Referral letter will be issued if there is a requirement to
follow up you at our clinics and referral letter can be obtained from,
•Medical officer/s at the OPD of the BH Gampola
•Medical officer of surrounding hospital
•Consultant of surrounding hospital
•If treated in ward at BH Gampola - ward doctor/Medical officer
You are advice to bring all the medical record and investigation report if you had
seek ant medical care earlier (either in the government sector or private
sector). This will avoid unnecessary investigation and delay.
Patients who are attending to clinic for the first time need to obtain a red
number. All other patients (subsequent visits) should obtain a blue number
including who treated in house at the BH Gampola from the respective clinic
record room.
To stay healthy you need to follow the dietary advices, positive life style
changes and to take prescribed medication correctly.
By doing the ordered investigation as instructed will allow us to provide you the
best available care.
